Some waterproof shelter (http://www.globalportablebuildings.com/Genesis.html will do just fine) to keep me and my stuff dry. A sealed booster box of Magic cards, a knife (preferably one of those big-ass survival knives, what we used to call a 'Rambo knife'), a d20, and a braille version of Stephen King's The Stand. No, I do not read braille, but I figure I will have plenty of time to try and teach myself. Plus, it will be something to read should I get old and go blind while staying there.
The bear is a substitute, in case either the shelter or the box of cards is not valid (since a box of cards technically isn't one item).

I'd take a solar rucksack to charge my iPhone full of neat apps, such as the Torch app. I would use my iPhone LED light to beam S.O.S out.
I would use the Swiss army knife to prepare dinner (whatever I catch/given) and then cook it on my BBQ, I would start it using rocks.
Since I am also on a remote island I am guessing I would want to explore so my DM's air-walk means I can walk for miles in comfort!
All of this I can carry in my sack.

I'm going to pretend that this island is like LOST and I find a magical bunker full of all the survival supplies I'm going to need. That way I can bring my iPod (the magic of the island will keep it charged), my dog, a book, my laptop (also charged by the smoke monster), and another person to listen to my constant chatter (my dog's a good listener, but she doesn't typically laugh at my lame jokes).
